Support Coordination
Support Coordination, within the context of the National Disability Insurance Scheme (NDIS), is a capacity building support that helps individuals understand, use and manage their NDIS plan.
Support Coordinators assist participants to connect with services, providers, and community resources so they can achieve their goals and live more independently.
They help individuals make sense of their NDIS plan, including what funding is available and how it can be used. Support Coordinators also help participants find and engage with service providers that meet their needs. They coordinate supports to ensure services work well together and contribute to positive outcomes.
Support coordinators build participants’ confidence in managing their own supports and encourage them to take an active role in their lives. They also act as a key point of contact, communicating with providers and advocating for the participant’s needs.
Support coordination is considered a reasonable and necessary support and is included in a person’s NDIS plan when extra help is needed to navigate the system, understand the plan or access appropriate services.
Psychosocial Recovery Coaching
Psychosocial Recovery Coaches (PRCs) work alongside participants to develop recovery focused plans that reflect their individual goals and needs. They support participants to understand and navigate the NDIS, helping them connect with the right services.
PRCs offer consistent emotional and practical support, encouraging engagement in the recovery journey. They also focus on building everyday skills and resilience to help individuals manage life’s challenges more independently.
PRCs assist with connecting to community and social activities and empower participants to understand their rights and speak up for their needs.
This support is especially beneficial for people with psychosocial disability or mental health conditions, those facing complex daily challenges, and individuals aiming to build independence, social involvement, and participation in work or study.
Social and Community Participation
Social and community participation is a vital part of living a fulfilling and connected life.
My Connectability supports individuals to engage in activities that match their interests and goals, whether that’s joining local groups, attending events, or building friendships.
By providing encouragement, practical assistance, and transport support if needed, we help participants overcome barriers to social inclusion and develop their confidence and skills. This involvement not only enhances wellbeing but also strengthens a sense of belonging and connection within the community.
Assistance with Daily Life
Assistance with daily life is essential for many individuals to maintain their independence and wellbeing.
My Connectability provides practical support tailored to each person’s needs, including help with personal care, meal preparation, household tasks, and managing daily routines.
Our goal is to empower participants to develop skills and confidence in managing everyday activities, ensuring they can live comfortably and safely in their own homes. This support is delivered with respect and compassion, focusing on enhancing quality of life and promoting greater autonomy.
SUPPORT
My Connectability provides a range of NDIS supports designed to help participants live more independently and reach their personal goals. Our housing supports, including Supported Independent Living (SIL) and Short Term Accommodation (STA)/Respite , offer safe, comfortable environments with assistance for daily routines, personal care, and household tasks, tailored to each individual’s needs.
Our Support Coordination team works alongside participants to help them understand their NDIS plans, connect with the right services, and ensure their supports are effectively managed.
We also offer assistance with daily living and social and community participation, supporting individuals with tasks such as personal care, meal preparation, cleaning, and accessing social or recreational activities. These supports are designed to build independence, confidence, and connection, helping participants to live fulfilling lives both at home and in the community.
Respite Care
Respite care is temporary housing support for participants thereby giving their carer a break. Respite care gives recipients a safe, comfortable environment and supportive environment.
Respite care can be for a few hours, a few days or longer depending on your individual needs.
NDIS respite care can include:
- Support with personal care and daily living tasks
- Assistance with medication or behaviour support needs
- Access to community or recreational activities
- Overnight or multi day stays in a supported setting (such as a group home or dedicated respite facility)
Supported Independent Living (SIL)
Supported Independent Living provides ongoing, daily support to help people with disability live as independently as possible in a shared or individual home setting. Funded under the NDIS, SIL is designed for participants who need significant support with everyday tasks and often includes 24/7 assistance, including overnight support if required.
SIL supports can include help with household tasks such as cleaning, laundry, and meal preparation, as well as personal care activities like bathing, dressing, grooming, and maintaining hygiene. Support workers may also assist with taking medication, developing routines, managing appointments, and building everyday living skills.
In addition to practical support, SIL promotes independence by helping participants build their confidence, make choices about how they live, and take part in their home and community life. The level and type of support provided is tailored to each person’s individual needs, goals, and preferences and is usually delivered in a shared living arrangement with other NDIS participants, though it can also be provided in individual settings where appropriate.
HOUSING
The NDIS provides funding for a range of housing-related supports, tailored to your individual needs and circumstances. This support is designed to help you live as independently and safely as possible in your chosen home environment. Depending on your situation, the funding may cover support workers coming into your home to assist with daily living tasks such as cleaning, laundry, meal preparation, personal care, gardening, yard maintenance, and other essential household activities. These supports are intended to reduce barriers to independent living and promote overall wellbeing. In some cases, more intensive support may be required. If you need ongoing assistance throughout the night, you may be eligible for sleepover or active overnight support. For participants with high support needs, the NDIS may also fund 24/7 support to ensure continuous care and safety. The level and type of support provided will be based on your functional capacity, goals, living arrangements, and what is considered reasonable and necessary under the NDIS.
